We offer primary care services by appointment only for patients ages 12 and up.  We do not currently offer Well Child Visits or Pediatric Vaccines.

As your PCP we will:

  • Help you manage chronic disease.
  • Help you develop an individualized healthcare plan.
  • Promote healthy aging through preventive care.
  • Detect changes in your health based on your history.
  • See the “big picture” of your overall physical and mental health.
  • Make referrals for further care.

What is a Primary Care Provider?

Also known as a PCP, this is a healthcare practitioner that is the first point of contact for patients’ medical
and healthcare needs. A PCP provides preventive health services, initial diagnosis and long-term
supervision of your non-emergency health care. PCP’s are advocates for the patient and coordinate the
use of the healthcare system to benefit the patient.
Having a Primary Care Provider is good for your physical and mental health.

  • Regular checkups with your PCP can help prevent chronic diseases.
  • Seven of the top ten deaths are caused by chronic diseases.
  • Primary care is a less costly option over Emergency Rooms

At your very first appointment we will do what is called an “establishing care” visit. This appointment is typically 30 minutes in length as we get to know each other.

What to expect at a Primary care appointment:

  • Plan to arrive 15-20 minutes prior to your appointment time.
  • If you are more than 10 minutes late you might be asked to reschedule.
  • Ensure your insurance and personal information is correct in our system.
  • Make your co-payment.

During regular Primary care visits:

  • Review your health since the last visit and update you chart.
  • Review any recent labs or tests.
  • Update any health screenings completed or order needed ones.
  • Discuss any new concerns you may have.

With primary care we like to see our patients in the office every 3-6 months depending on their individual needs. If you are taking chronic medications every 3-6 month in person visits is a good standard of care.
